Unblocking celly account?

my mom is being a ho and suspended my account or blocked it or whatever she did so i cant make or recieve any calls or txts. The only thing i can call is verizon so I need to call up and get it unblocked quick . All they have to do is push a button and it works again. The problem is that account is under her name so what do I need other than name, birthdate etc? social security number?
