I assume you are referring to nn-dmt as that is what most people are thinking of when they say dmt. I did a decent amount of it last summer, like 25-30 or so times. Just like most drugs, it's really tough to explain the effect's in a way that conveys them well. Basically you either "break through" or don't. Think of a big wall, the farther up you climb the more you start to notice small patterns dancing on your floor or something, but it isn't till you reach the top and are able to look over to the other side that you have the full effects set in. What causes someone to break through or not I don't know, but the freshness of the substance and the person's willingness to not fight it are definitely important. Each time I have done it my body feels like it's vibrating, not like any other drug I've done, and because of the short duration of effect's the onset is very rapid. I would experience this sort of build up of energy starting from inside my body and it would expand outward until I would "break through". I won't touch on the visuals as there is no way i could possibly type anything close to what you can experience, but go watch enter the void and you will have a very very very rough idea of what you might see. Infinite geomorphic shapes and patterns and colors. DMT is a very powerful substance, I wouldn't consider it a hard drug by any means though, as it is not something you abuse, it is a tool to see into one's self.
