^yep thats what it sounds like. Your foot is probably pronating causing your ankle bone to press on the inner shell. A good custom footed with possibly a stretch to the ankle may help. Dont get any stretching done without a footbed though as your foot may just pronate more into the extra space.

Best bet is to go find a good bootfitter, have him look at the fit of the boots and see what he recommends. Without seeing you in the boots we are all just guessing.

your boots are likely to big.

If you cant afford new boots you could try replacing the liners with new ones, or add a "booster strap" to them, or add "super feet" insoles to them.

Its not actually lickly that his boots are too big. If they were too big there would be extra space inside the boot and his problem may not be as bad, but he would have many other problems.

Its much more lickly to be a support issue, but as I said, he needs to take them to a shop, not just spend money on new liners or boosters. And defiantly dont get Superfeet, get a good custom footbed, not a premade insole.

Get boots that fit/Custom insert mould. I had a pair of Lange racing boots, and the inners were really unforgiving in those, I think Lange's need to be exactly the right size otherwise they can be really uncomfortable.
