When that shit happens to my nails I take a small (sharp) drill bit and slowly drill through the nail, using my free hand slowly spinning it with my fingers, to releive the pressure. It is the best feeling when that blood comes out.

Mine was super bad, my suitcase fell on my toes, the nail turned like blue/purple, and then eventualy black. It was the worst feeling in the world, i should of drained it.

i don't know if your toenail will in fact fall off, but that is exactly what my mom's looked like. hers also turned darker over time so if that is a picture of very soon after your toenail got injured and it gets darker, bad news! but, if that's a picture of the nail after a few days you may be ok. but what do i know? i am no doctor.
