Done, i had to do similar surveys for my sociology courses the last few years. Wasnt on NS at the time though, good luck.

I suggestion for you if you ever make more surveys for other classes. Make sure you have a brief description at the beginning of the survey telling your participants what your survey is about and why you are conducting the survey. Also, leave those demographic questions (age, gender, etc.) for the end of the survey because participants don't like questions like that at the beginning. Also, when you have a long list of options like your satisfaction question, list them left to right. It's been shown that survey takers like to read left to right (pretty obvious) but they lose interest if they have to read a bunch of options going down the page.

I had a marketing research class last semester in school and those are the tips that I learned when constructing surveys. Not sure if you'll need this information, but hopefully it helped in some way.

Thank you so much, and I'll keep your comments in mind for the next time I have to make a survey.

And for a little bit of background on what it's for, we had to create a survey assessing the usage of the iPod/ iPod app on different apple devices. The survey was to be limited to 10 questions at max and is an exercise in using the software that was used to create the survey.
