Doctors See a Big Rise in Injuries for Young Athletes

By BILL PENNINGTON

Published: February 22, 2005

A competitive swimmer since she was 7, Alex Glashow of Barrington, R.I., logged 8,000 yards a day in the pool, until her arms ached. She learned to dislocate one shoulder intentionally to ease the pain in the water, but after shoulder surgery and a year of physical therapy, Glashow quit competitive swimming forever when she was 15.

Jeret Adair, a top young pitching prospect from Atlanta who started 64 games in one summer for his traveling baseball team, last year had Tommy John surgery, an elbow reconstruction once reserved for aging major leaguers.

Ana Sani of Scarsdale, N.Y., a 13-year-old budding soccer star, practiced daily until she tore the anterior cruciate ligament in her knee.

Around the country, doctors in pediatric sports medicine say it is as if they have happened upon a new childhood disease, and the cause is the overaggressive culture of organized youth sports.

'They are overuse injuries pure and simple,' Dr. James Andrews, a nationally prominent sports orthopedist, said. 'You get a kid on the operating table and you say to yourself, 'It's impossible for a 13-year-old to have this kind of wear and tear.' We've got an epidemic going on.'

Typical injuries range from stress fractures, growth plate disorders, cracked kneecaps and frayed heel tendons to a back condition brought on by excessive flexing that causes one vertebra to slip forward over another vertebra. Most are injuries once seen only in adults.

Dr. Lyle Micheli, a pioneer in the field of treating youth sports injuries and director of the sports medicine division of Boston Children's Hospital, said that 25 years ago, only 10 percent of the patients he treated came to him for injuries caused by overuse. Back then, most childhood injuries were fractures and sprains. Dr. Micheli said overuse injuries now represented 70 percent of the cases he sees. In interviews with more than two dozen sports-medicine doctors and researchers, one factor was repeatedly cited as the prime cause for the outbreak in overuse injuries among young athletes: specialization in one sport at an early age and the year-round, almost manic, training for it that often follows.

'It's not enough that they play on a school team, two travel teams and go to four camps for their sport in the summer,' said Dr. Eric Small, who has a family sports-medicine practice in Westchester County. 'They have private instructors for that one sport that they see twice a week. Then their parents get them out to practice in the backyard at night.'

Pushing Children to Overachieve

Dr. Angela Smith, an orthopedic surgeon at the Children's Hospital of Philadelphia, said parents in virtually every sport were pushing their children to excess in pursuit of college scholarships or the dream of a professional sports career. 'The volume of training has increased beyond the maturing young body's ability to handle it,' she said.

Doctors lament the loss of what has become a cultural artifact: the playground athlete. Two decades ago, sports for children were often unorganized, with pick-up games common in schoolyards and community parks.

'Children might have played baseball, basketball and football all in the same day,' Dr. Micheli said. 'This was good for their bodies, which developed in balance. Now young athletes play sports supervised by adults who have them doing the same techniques, the same drills, over and over and over.

'By playing one sport year-round, there is no rest and recovery for the overused parts of their body. Parents think they are maximizing their child's chances by concentrating on one sport. The results are often not what they expected.'

In his office in Birmingham, Ala., Dr. Andrews hands the parents of new patients a piece of chalk and points to a blackboard in the corner.

'I say, 'Write down when your child started playing his sport, how many teams he's played for, what camps he went to, for how many years, what private instructors he's seen, what championships he won, what his stats were, all that stuff,' ' Dr. Andrews said. 'Then I walk out of the room. I come back in and they've filled up the blackboard. They're proud.

'And I say, 'You all know why he's here seeing me?' And I point to the blackboard. That's when the light bulb goes off.'

Well if you think about it, with the extra press for professional sports going on, more and more adults are pushing their kids to suceed. In almost every case, that pushing is too hard and too much stress for the kids' small frames to handle.

my knees ache everyday pretty much all day, they have since i was in my senior year of high school. i am now 24. all the years of basketball and skiing probably destroyed most of the cartiledge in my knees. i wont go to the doc for it cause i dont wanna hear you cant ski or do this or that anymore.

I'm scared that by the time I'm 40 my knees wont be there for me. Our parents never did any of the shit we do skiing. The most wear and tear their knees felt was jumping out of a tree. Coming up short or overshooting a table probably does more damage than a week of running. Do it two or three times a day and you're shit is going to seriously be fucked. Moguls especially. Plus my back is fucked. I've got this thing where one of the muscles is only partially attched to the bottom ribs. According to the doctor it's supposed to be an overuse injury. I took a jump to big when I was 11 years old and it's hurt madly for a month a year every year since. It's debilitating. Fortunatly, it almost always hurts in the fall when I'm training for skiing and not during the actual ski season itself. I should stop but I cant and I'm going to pay for it.

A pitcher started 64 games??? If that's not healthy for a professional athlete who's in peak athletic form, how can that be healthy for a growing kid. It sucks that people can't think. In the context of skiing, I'd imagine that wear and tear is greatly magnified, especially when landing long or short (as stated earlier). I think in most cases for skiing though, people know what they're doing to themselves, they just don't care. I have more sypmathy for the kids who are just going along with their parent's encouragement.

haha, either way a child was raised they are going to be messed up. Sitting inside playing video games and becoming super fat, or going outside and playing real games and becoming disabled. Good thing I can play tetris while walking!

Yeah, i agre that our generation is going to drain the health care, if you arnt constantly getting injured by being active, you are fat, too few kids live a balanced lifestyle, kids at my school who are 14 are on creatin, its disgusting.

I have a torn meniscus in my knee from getting up off the ground, and turning at the precise angle and at the exact time that would hurt me. I have bad luck

more people get hurt because the level that sports are at is pretty intense. like 10 years ago no one would be doing rodeos of cliffs.

,',',',',',',',',',',',',',',',',',

im not very messed up, and ive played football for five years, and skiing for what would be my tenth year if we could get some snow. lower backs a little messed, ankles, knees, and groin a little. that sounded sarcastic, but it wasnt.

I don't deny there are bad things in the US right now, hell, 51% of the country to be exact. But god damnit, our country being fat is NOT a problem. I do't give a shit how fat people are. hell, fat kids are harder to kidnap, that means our country is safer.

The issue isn't how long you've been playing, it's how often you play.

Com'on just because your shit aches does mean your future athletic capability is permanently impaired.

Bodies are growing, joints settling, plus you're abusing them. Alot of this is just common wear and tear. Granted alot of us are more active than your average couch whore, but hell...

I've broken: my arm, elbow, toes, both thumbs real bad, messed up back to the point of bed riddance, torn ankle ligaments, sprained knees, bruised bones, broken my leg, snapped my humerus, concussions, two major surgeries, the whole gammit.

But I'm still chugging along.

Yes, those kids were drastically over-worked and adhered to far too intensive a workout regiment. The point is, listen to your body, it knows best, take care of it - rest, stretch, meditate, ice, massage, hot bath whatever you need just do what works. You won't get anywhere by just shrugging off pain. You gotta work with the pain. And after countless hours in phys ther, thats something I know.

ive got the problems with knee, hip, back, shoulder, elbow, wrist fuck... ive got it all. but most of its not that bad. i went to physical therapy for a while for my knees but i didnt think it was doing much so i quit doing it. but between skiing and year round training my body's probably just worn out a bit. thats the problem with kids in sports though, we dont want to stop and give our bodies time to recouperate because if you do you lose strength that you gained training/practicing. so you just keep going basically non stop.
