well pretty simple actually... ski up to the jump with sufficient speed, and just throw a front flip, but while you're in the air flip 3 more times.
 
Based on science, on a 15 foot jump you'll need to be going approximately 3 billion miles an hour, you'll get it easy.

 
3 billion mph. That is 4.47349479609 times faster than the speed of light (actually). How in the living fuck can that be accomplished? I mean, yes, with a wormhole you can travel great distances with an average speed faster than the speed of light, but you aren't actually going faster than the speed of light, you are just skipping distance while traveling at a speed less than the speed of light (relative to the wormhole). So your actual speed (internal distance/time) is less than the speed of light, while your average speed (external distance/time) will be greater than the speed of light with a wormhole.

In other words, you can't actually exceed the speed of light. Which means over 4 times less speed for the jump. So you must set the front flip much harder. If the jump has 2 seconds of hang time, make sure your angular velocity of the flip is greater than 740°/ second (accounting for the upward angle of the jump and downward angle of the landing). Huck and tuck, the harder you tuck, the greater the angular velocity.
