Is my mount completely screwed?

I got pivot 15s mounted on my wets, and this was my 12th day on the mountain with them. I noticed that my ski was loose on my foot, and checked out what was going on. The half moon piece is not flush to the rest of the binding, and it looks like the entire turntable is popped out a little as we can see in the picture.

Has anyone else seen something like this? Is this a binding failure or a bad mount?

I'm not particularly heavy, around 155lbs. I am also not skiing incredibly aggressively, so breaking a pivot after such a short amount of time is very surprising. Any thoughts?

Put a small screwdriver or something in there and just pull the tab back, rest should fall into place

Aren't all of those parts sandwiched under the back mounting screws? I'm concerned about whether there's an issue with that, because if those pieces are coming apart, it seems like the screw is loosey goosey too? I may not understand the way pivots work though :/

It should be alright, the white tab is the forward pressure indicator and works by push against the rest of the binding. It definitely feels a little awkward pushing it back into place, but that's most likely what's going on.

I've had that tab get pushed under the heel parts on my own and the screws weren't loose

Update: was able to get under the white tab like you said thank you! The half moon piece is still "flipped back" when it should be flush, do I just need to replace that part?

rotate the heel to the side and back. Sometimes mine do this where the white tab makes its way under. If you think the heel screws are loose then take the ski to the shop that mounted them and ask.
