Shes probably seeing it for the first time on the teleprompter as shes reading it with no reason to second guess it. Whoever got the info probably didnt think about it, say it out loud, and fed it through. I can see how it happened but still one of those stars align deals for nobody to notice it.

I can't imagine being like oh fuck, after reading it, and then having to apologize and look like a total idiot.
 
I have always had a hard time processing this video. Did someone just say "fuck it" and get themselves fired by writing in made up names to the script? Is this just a fake video for internet views? Or did this actually occur somehow in this strange universe?
